24-Hour Community Mental Health

Team Wellness Center offers 24-Hour Community Mental Health (CMH) services. This program provides treatment to people with mental illness and substance use disorders while addressing the social determinants that adversely affect their recovery. The goal of the 24-hour program is to improve the mental, physical, emotional, and economic well-being of participants by providing access to treatment services around the clock.

Team Wellness Center’s 24-Hour CMH program offers comprehensive services so support individuals with behavioral health issues, including:

• Mental Health Treatment
• Substance Use Treatment
• Individual and Group Therapy
• Peer Mentorship
• Employment, Skill Building & Pre-Vocational Training
• Adult Education / G.E.D. Classes
• Dental Care
• Primary Care Services
• Housing Assistance
• Food & Accommodations (as available)

Eligibility & Requirements

• Team Wellness Center does not operate a shelter. To be eligible for our 24-Hour CMH program, individuals must have one or more of the following behavioral health issues at the time they enroll:
  • Mental Illness
  • Substance Use Disorders
  • Developmental Delays

• Members participating in the 24-Hour CMH program are required to:
  • Be assigned to and work with a treatment team
  • Develop an individualized plan of service
  • Participate in treatment and therapy in accordance with their plan
  • Adhere to Team Wellness Center and 24-Hour CMH program rules
